RESUMO

Abstract To evaluate the microtensile bond strength (µTBS) of two resin cements to 3D printed and milled CAD/CAM resins used for provisional fixed partial dentures. Blocks (5 x 5 x 5 mm) of three 3D-printed resins (Cosmos3DTemp / Yller; Resilab3D Temp / Wilcos and SmartPrint BioTemp, / MMTech) were printed (Photon, Anycubic Technology Co.). A milled material (VitaCAD-Temp, VITA) was used as control. Half the specimens were sandblasted and the rest were untreated. Two blocks were bonded with the corresponding resin cement: PanaviaV5 (Kuraray Noritake) and RelyX Ultimate (3M Oral Care). After 24 hours, the bonded blocks were sectioned into 1 x 1 mm side sticks. Half the beams were tested for µTBS and the other half was thermocycled (5000 cycles, 30s dwell-time, 5s transfer time) before µTBS testing. A four way Generalized Linear Model (material*sandblasting*cement*aging) analysis was applied. VITA exhibited the lowest µTBS, regardless of the cement, sandblasting and thermocycling. Sandblasting significantly improved the µTBS of VIT, especially after aging, but did not improve the µTBS of 3D printed resins. Sandblasting was not beneficial for 3D printed resins, although is crucial for adhesive cementation of milled temporary resins. Airborne particle abrasion affects the integrity of 3D-printed resins, without producing a benefit on the microtensile bond strength of these materials. However, sandblasting is crucial to achieve a high bond strength on milled temporary resins.

Introduction: Indirect resin restorations are widely used in the esthetic treatment of posterior teeth. Preheated resins have been recently proposed as luting agents. Objective: Evaluate the tensile bond strength of indirect restorations cemented with a preheated resin versus a self-adhesive resin cement and a resinous cement. Methods: Tooth preparation was conducted of 45 premolars using calibrated diamond-coated stones. The samples were randomly divided into three sets (n = 15) according to the following experimental groups: Group 1 (preheated resin), Group 2 (self-adhesive resin) and Group 3 (adhesive resinous cement), and soaked in self-polimerizable acrylic. Indirect restorations were performed for each sample. The inner surfaces were microsanded, and silane was applied before cementation with the luting agents. Tensile bond strength was verified with a universal testing machine at a crosshead speed of 5 mm/min. Statistical analysis was based on one-factor ANOVA and Tukey's post hoc test. A significance level of p < 0.05 was established. Results: The Filtek Z250 XT resin preheated at 60°C for 15 minutes displayed a tensile bond strength of 5.775 MPa, similar to RelyX Ultimate with 5.442 MPa (p > 0.05). Both groups displayed a significantly greater tensile bond strength than RelyX U200 (3.430 MPa). Conclusions: The preheated resin (Filtek Z250 XT) and the resinous cement (RelyX Ultimate) exhibit the same tensile bond strength properties when used as luting agents in indirect resin restorations(AU)

RESUMO

Abstract: This study aimed to evaluate the bond strength of a universal adhesive system to enamel surrounding real-life carious cavities. Twenty-eight permanent molars (n = 7) with carious lesions in dentin were subjected to selective carious tissue removal to firm dentin and had their crowns sectioned longitudinally. A universal adhesive system (Single Bond Universal [SBU] used in either etch-and-rinse and self-etch strategies) was compared with an etch-and-rinse Adper Single Bond 2 (ASB) and a self-etch Clearfil SE Bond (CSE) adhesive systems (control systems). Adhesives were applied on the enamel, assumed demineralized, surrounding the cavity margins and on sound enamel (control substrate). Composite cylinders were built (0.72 mm2) and microshear bond strength (µSBS) test was performed after 24 h of water storage. The µSBS values (MPa) were analyzed using two-way ANOVA and Tukey's post hoc tests (α = 0.05). Bond strength values obtained in demineralized enamel surrounding carious cavity margins were significantly lower than that obtained in sound enamel (distant from carious cavity margins) (p = 0.035). The bonding strategy of the SBU did not influenced the bond strength values, which were higher than that obtained with ASB. CSE showed similar µSBS values to ASB and SBU in the self-etch mode. In conclusion, the bond strength to enamel assumed demineralized is lower than to sound enamel. The enamel surrounding carious cavities jeopardize the bonding of universal adhesive system. The bond strength of universal adhesive is similar, regardless to bonding strategy.

RESUMO

Abstract Resin cements are often used for single crown cementation due to their physical properties. Self-adhesive resin cements gained widespread due to their simplified technique compared to regular resin cement. However, there is lacking clinical evidence about the long-term behavior of this material. The aim of this prospective clinical trial was to assess the survival rates of metal-ceramic crowns cemented with self-adhesive resin cement up to six years. One hundred and twenty-nine subjects received 152 metal-ceramic crowns. The cementation procedures were standardized and performed by previously trained operators. The crowns were assessed as to primary outcome (debonding) and FDI criteria. Statistical analysis was performed using Kaplan-Meier statistics and descriptive analysis. Three failures occurred (debonding), resulting in a 97.6% survival rate. FDI criteria assessment resulted in scores 1 and 2 (acceptable clinical evaluation) for all surviving crowns. The use of self-adhesive resin cement is a feasible alternative for metal-ceramic crowns cementation, achieving high and adequate survival rates.

There are many factors that influence the success of cosmetic treatment for the closure of a central diastema, because of the multiple causes to the presence of this entity. Among them, the traumatic avulsion of one of the central incisors is one of the largest and most complex problems to be solved. The objective is to describe a case of closure of a central maxillar diastema to restore the aesthetic appearance of a patient, using resin as a restorer nano-particle material. As part of the treatment to a male 25-year-old patient with mild mental retardation and history of dental injury with avulsion of the right 1.1 maxillary central incisor, the closure of the upper central diastema was performed with the application of nano-particle resin, prior to preparation of biomechanics and acid application. The technique used constitute is a therapeutic alternative that greatly benefits the patient. With the sequence of the proposed steps, which offers numerous advantages compared to that presented by other authors in similar cases, the aesthetic appearance of the patient was reestablished after just one visit by the appropriate clinical diagnosis and thorough a planned treatment(AU)

Hybrid lithium disilicate custom abutments are devices that adjust the shape and the emergency profile of the implant supported restorations in each clinical situation, getting good optical characteristics and the possibility of adhesive cementation. The purpose of the study was to compare the fracture strength of Ti-base Hybrid lithium disilicate and yttria-tetragonal zirconia custom abutments through an experimental in vitro study. Ten abutments were prepared, five as a control group made in Y-TZP by multi-system X Ceramill Amann Girrbach®, and five lithium disilicate hybrid abutments by heat press technique (IPS e.max® press abutment solution, Ivoclar Vivadent) as experimental group. Each abutment was preloaded to an implant with torque of 30 N/cm2. The crowns were fabricated in lithium disilicate (monolithic technique) and cemented using dual-curing resin cement, RelyX U200, 3M ESPE. The fracture strength measurements were performed on a universal testing machine (instron®), stored and evaluated using the t-student parametric test. An optical microscope (Carl Zeiss) was used to evaluate the incipient fractures and classify the type of failure. The average fracture strength of Ti-base Hybrid lithium disilicate was 655.5 N and the value for yttria-tetragonal zirconia custom abutments was 780.52 N, with a standard deviation of 145.76 N and 147.71 N, respectively. In both cases there was no statistical difference. Both types of abutments showed good performance to fracture strength test. Nevertheless more studies are required to evaluate this type of abutments.

The marginal adaptation is one of the basic requirements for the longevity and success of restorations. The aim of this study was to evaluate the influence of the internal imbalances associated with different luting agents on marginal leakage of crowns Nanoceramic resin ( Lava Ultimate 3M ESPE) made by CAD-CAM system. From the scanning of a full crown preparation in a pre molar were obtained 36 metal models. These were divided into six groups according to the internal imbalances ( 70 or 100 micrometres) and cement (SS White Zinc phosphate, glass ionomer cemet Ketac Cem 3M/ Espe and resinous cement Relyx Ultimate 3M / Espe). After scanning all models crowns were made from milling Nanoceramic resin blocks (Lava Ultimate 3M / Espe). With the aid of optical comparator microscope (40X) were measured prior marginal mismatches and after the cementation, being considered 8 points along the end in all specimen faces. The values were submitted to ANOVA two-way test and t test. Results showded that crowns made with internal imbalances of 70 uM show statistically significant lower marginal leakage to the crows with internal imbalances of 100 micrometres. For the internal imbalances 70 uM zinc phosphate cement group showed the lowest "gap" marginal, and this figure does not show a statistically significant difference compared to Ketac Cem gropu. Even with this misfit, Ketac Cem group showed no statistically significant difference in relation to Relyx Ultimate group, though the latter showed a statistically significant difference compared to zinc phosphate cement group. For the internal imbalances of 100 micrometres there was no statistically significant difference in the "gap" marginal among the three cements. This study demostrated through the methodology used, the lower marginal leakage oberved in Lava Ultimate restorations, was from the inner misfit 70 micrometers...

RESUMO

Marginal integrity is one of the most crucial aspects involved in the clinical longevity of resin composite restorations. Objective To analyze the marginal integrity of restorations produced with a model composite based on polyhedral oligomeric silsesquioxane (POSS).Material and Methods A base composite (B) was produced with an organic matrix with UDMA/TEGDMA and 70 wt.% of barium borosilicate glass particles. To produce the model composite, 25 wt.% of UDMA were replaced by POSS (P25). The composites P90 and TPH3 (TP3) were used as positive and negative controls, respectively. Marginal integrity (%MI) was analyzed in bonded class I cavities. The volumetric polymerization shrinkage (%VS) and the polymerization shrinkage stress (Pss - MPa) were also evaluated. Results The values for %MI were as follows: P90 (100%) = TP3 (98.3%) = B (96.9%) > P25 (93.2%), (p<0.05). The %VS ranged from 1.4% (P90) to 4.9% (P25), while Pss ranged from 2.3 MPa (P90) to 3.9 MPa (B). For both properties, the composite P25 presented the worst results (4.9% and 3.6 MPa). Linear regression analysis showed a strong positive correlation between %VS and Pss (r=0.97), whereas the correlation between Pss and %MI was found to be moderate (r=0.76).Conclusions The addition of 25 wt.% of POSS in methacrylate organic matrix did not improve the marginal integrity of class I restorations. Filtek P90 showed lower polymerization shrinkage and shrinkage stress when compared to the experimental and commercial methacrylate composite.

OBJECTIVE: To demonstrate the application of the modified Ottawa method by establishing the update need of a systematic review with focus on the caries preventive effect of GIC versus resin pit and fissure sealants; to answer the question as to whether the existing conclusions of this systematic review are still current; to establish whether a new update of this systematic review was needed. METHODS: Application of the Modified Ottawa method. Application date: April/May 2012. RESULTS: Four signals aligned with the criteria of the modified Ottawa method were identified. The content of these signals suggest that higher precision of the current systematic review results might be achieved if an update of the current review were conducted at this point in time. However, these signals further indicate that such systematic review update, despite its higher precision, would only confirm the existing review conclusion that no statistically significant difference exists in the caries-preventive effect of GIC and resin-based fissure sealants. CONCLUSION: In conclusion, this study demonstrated the modified Ottawa method as an effective tool in establishing the update need of the systematic review. In addition, it was established that the conclusions of the systematic review in relation to the caries preventive effect of GIC versus resin based fissure sealants are still current, and that no update of this systematic review was warranted at date of application. .

RESUMO

The aim of this study was to evaluate the effect of treating intraradicular dentin with irrigating solutions on the retention of glass-fiber posts luted with self-adhesive resin cement. Bovine incisors were endodontically treated, and 9-mm-deep postholes were prepared. Before inserting the cement, the root canals were irrigated with various solutions: 11.5% polyacrylic acid for 30 s, 17% EDTA for 60 s, or 5% NaOCl for 60 s, respectively. Irrigation with distilled water was used in the control group. After all specimens had been rinsed with distilled water, the excess moisture was removed and the posts were luted using either BisCem (Bisco) or RelyX Unicem clicker (3M ESPE). Seven days after luting, the specimens were sectioned transversally into 1-mm-thick slices, which were submitted to push-out testing on a mechanical testing machine. Bond strength data (n = 6 per group) were analyzed by two-way ANOVA and Student-Newman-Keuls' test (α = 0.05). For Unicem, EDTA showed lower bond strength than the other solutions, which had similar results. For BisCem, EDTA showed higher bond strength than the other treatments, while application of NaOCl yielded higher bond strength than polyacrylic acid whereas the control group had intermediate results. In conclusion, irrigating root canals before insertion of self-adhesive resin cements, especially EDTA, might interfere with retention of the fiber posts.

RESUMO

OBJECTIVES: The objective of this one-year clinical study was to investigate the effect of two adhesive systems (Adper Single Bond, a two-step etch-and-rinse and Clearfil SE Bond, a two-step self-etch system) on pit-and-fissure sealant retention in newly-erupted teeth. This study compared the success of the sealants in mesial and distopalatal grooves with and without these two adhesive systems. MATERIAL AND METHODS: In a clinical trial, 35 children aged 6-8 years undergoing sealant placement were recruited. This one-year clinical study scored 70 mesial and 70 distopalatal sealants of newly-erupted permanent maxillary first molar, with a split-mouth design. All children received sealant alone in one permanent maxillary molar tooth. Children were randomized into two groups. One group received Self-etch (Se) bond plus sealant and the other group received Single Bond plus sealant in another permanent maxillary molar tooth. Clinical evaluation at 3, 6 and 12 months was performed and the retention was studied in terms of the success and failure. RESULTS: The success rate of sealant in the distopalatal groove, using SeB at 3, 6 and 12 months was 93.3% (95% CI: 68.0, 99.8), 73.3% (95% CI: 44.9, 92.2) and 66.7% (95% CI: 38.4, 88.2), respectively. It was greater than that of the distopalatal groove in SB group with a success rate of 62.5% (95% CI: 35.4, 84.8), 31.3% (95% CI: 11.8, 58.7) and 31.3% (95% CI: 11.8, 58.7), at the three evaluation periods. The success rate of sealant in the mesial groove using SeB was 86.6% (95% CI: 59.5, 98.3), 53.3% (95% CI: 26.6, 78.7) and 53.3% (95% CI: 26.6, 78.7), while this was 100% (95% CI: 79.4, 100.0), 81.3% (95% CI: 54.4, 96.0) and 81.3% (95% CI: 54.4, 96.0) using SB, at 3, 6 and 12-month evaluation periods. CONCLUSIONS: These results support the use of these two bonding agents in pit-and-fissure sealants under both isolated and contaminated conditions. Further, Se bond seemed to be less sensitive to moisture contamination.

RESUMO

OBJECTIVES: Adhesive systems are continuously being introduced to Dentistry, unfortunately often without sufficient clinical validation. The aim of this study was to evaluate the clinical performance of cervical restorations done with three different adhesive systems. MATERIAL AND METHODS: 158 non-carious cervical lesions of 23 patients were restored with a nanofilled composite resin (Filtek Supreme, 3M/ESPE) combined with Single Bond (3M/ESPE, group SI), Clearfil SE (Kuraray Medical Inc., group CL) and Xeno III (De Trey Dentsply, group XE). In groups SI-B, CL-B and XE-B, the outer surface of the sclerotic dentin was removed by roughening with a diamond bur before application of the respective adhesive systems. In groups CL-BP and XE-BP, after removal of the outer surface of the sclerotic dentin with the bur, the remaining dentin was etched with 37% phosphoric acid and the self-etch adhesive systems Clearfil SE and Xeno III were applied, respectively. Lesions were evaluated at baseline, and restorations after 3 months, 1 year and 2 years using modified USPHS criteria. RESULTS: After 2 years, no significant difference was found between the retention rates of the groups (p >0.05). Although groups CL and SI showed significantly better marginal adaptation than group XE (p<0.05) at 2 years, no significant difference was found between the marginal adaptation of the groups SI-B, CL-B and XE-B (p>0.05). After 2 years no significant difference was observed among the marginal staining results of all groups (p>0.05). CONCLUSION: Although all adhesive systems showed similar retention rates, Clearfil SE and Single Bond showed better marginal adaptation than Xeno III after 2 years of follow-up.

The effect of post-space treatment whit EDTA on the bond strength (BS) of fiber posts in different root regions was evaluated using two different type of resin cements Rely X U100 (UC) and Para Post Para Core automix (PC) . Fourty extracted premolars root canals were assigned to four groups of 10 roots each. G1 RelyX U100 (UC) according to manufacturer's instructions (MI), G2 etching whit EDTA 24% for 60 s before application of UC; G3 Para Post Para Core Automix (PC) MI; G4 Etching whit EDTA 24% for 60s before application off PC. After cementation the roots were sectioned in 6 slices 1mm each (two slides for root region) and randomly divided into two subgroups, depending on testing time immediate (IM) vs. after termocicling(TM) ), for expulsion test. Statistical analysis was performed with ANOVA and Tukey test (?=.05) No differences in BS was found among the groups whit and whit out EDTA 24 %. Etching whit EDTA adversely alter the BS values of the cervical third in the two cements. When TM was compared with IM with or without application of EDTA there was no statistically significant difference. For all groups tested, PC show higher values of BS (19.59 MPa) was compared with UC (15.80 MPa). Concluded that treatment with EDTA affected the BS only on the cervical third in both IM and TM in the two cements. The TM with or without EDTA did not affect the values of BS when compared with IM

RESUMO

Predicting the prognosis of molars that have experienced furcation invasion, is often a frustrating experience to the dental clinician and disappointing report to the patient involved. Although multiple treatment modalities have been attempted to retain teeth with severe furcation invasion, clinical success has not been predictable. A case report involving the use of glass ionomer cement (GIC) as an occlusive barrier in the management of Class III furcation defect involving mandibular first molar is presented. A literature review on the subject matter was conducted using Medline, Google search engines, and manual library search. GIC restoration of Class III furcation invasion gives a satisfactory result. Surgical and nonsurgical treatment options are available for the management of the condition. GIC as an occlusive barrier in Class III furcation invasion is an economical and less invasive treatment option. It also makes home care easy for the patient.

OBJECTIVE: This study assessed in vitro how the chemicals used in the preparation of root canals affected the force required to remove the resin cement Rely-X (3M, Sumaré, Brazil) from bovine root dentin. METHODS: The crowns of 72 bovine teeth were sectioned and the roots were standardized. The specimens were then placed in resin blocks and treated for 10 minutes with sodium hypochlorite at 0.5 percent, 1 percent, 2.5 percent or 5.25 percent or chlorhexidine gel at 2 percent. The control group was treated with distilled water. Next, the specimens were soaked in 17 percent EDTA for 3 minutes. The surfaces of two specimens from each group were assessed by electron scanning microscopy. The tested specimens were then treated with 37 percent phosphoric acid for 15 minutes, adhesive system Single-Bond (3M, Sumaré, Brazil) and the resin cement Rely-X (3M, Sumaré, Brazil) was applied over it. RESULTS: The results in Kgf were: GChlorhex (2.26); GNaOCl-0.5 percent (7.92); GNaOCl-1 percent (9.70); GNaOCl-2.5 percent (9.05); GNaOCl-5.25 percent (8.91); GA (0.61). CONCLUSION: Sodium hypochlorite in the tested concentrations increased the adhesion of the resin cement to the dentin significantly when compared with water and 2 percent chlorhexidine. Electron scanning microscopy showed that the structure of the root dentin treated with sodium hypochlorite presented widened dentinal tubules, less intertubular dentin and depression areas, all of which were not observed in the roots treated with water and 2 percent chlorhexidine gel.

Root canal therapy in teeth with large coronal destruction is a common clinical precedure in the daily dental clinics. In most of these conditions, there is an indication of the use of intra-radicular post for stability and retention for the restoration. One of the techniques proposed for the treatment of enlarged channels, is the use of a fiber glass with modulus of elasticity similar to dentin, restored with composite resin to fill the space between the root canal extended. This technique reduces excessive amounts of cement that would serve to replace the lost tooth structure and allows a good adaptation in the root canal, creating favorable conditions for retention of the post. The objective was to demonstrate the technique of anatomization intraradicular aesthetic post with composite resin in anterior incisor with large coronal destruction. The use of a fiber glass anatomical was the alternative used for clinical resolution in order to reinforce the remaining tooth structure weakened, returning him to function without requiring the removal of sound tooth structure, and aesthetics combined with a more favorable prognosis.

RESUMO

Background and Objectives: One of the most difficult problems encountered in orthodontic treatment with fixed appliance is the control of enamel demineralization around the brackets. Fluoride-releasing bonding adhesives were introduced to aid in the prevention of demineralization adjacent to orthodontic brackets. Hence, an in vitro study was conducted to evaluate and compare demineralization adjacent to the orthodontic bracket and the bond strength using fluoride-releasing adhesive and conventional adhesive. Materials and Methods: One hundred and twenty healthy extracted premolars were selected for the study and were divided into two groups, each group consisting of 60 teeth. Both groups were further divided into three subgroups consisting of 20 samples each. Samples of subgroups A and B were bonded with two prototypes of fluoride-releasing adhesives (i.e. Rely-a-bond and Tru-bond). Samples of subgroup C (ortho-one) were bonded with conventional non-fluoride-releasing adhesive. Group I samples were subjected to demineralization in an acidic medium (methyl cellulose buffered with acetic acid at a pH of 4.5) for 4 weeks, which were later observed under a stereomicroscope. Group II samples were tested for shear bond strength using the Instron Universal testing machine. Results: Both fluoride-releasing adhesives showed statistically significant lower enamel demineralization around the bracket when compared with the conventional adhesive. There was no statistically significant difference on comparing the demineralization between the two prototypes of fluoride-releasing adhesive. Evaluation of shear bond strength revealed that there was no significant difference between the subgroups. Conclusion: Fluoride-releasing adhesives might aid in the prevention of demineralization adjacent to the orthodontic bracket with acceptable bond strength.

RESUMO

Background: Dentin hypersensitivity is a recurrent condition causing discomfort and sometimes pain to the patient, which also deters him from maintaining adequate oral hygiene. Home care and office measures are used for treatment of this malady. Aim: To evaluate and compare the efficacy of 2 treatment modalities for dentinal hypersensitivity, iontophoresis with acidulated phosphate gel (APF) gel, and dentin-bonding agent application. Materials and Methods: This split mouth randomized clinical study recruited subjects with a history of hypersensitivity with at least 2 teeth, verified by a light stroke with a dental explorer along the cervical third of the teeth. The patients were subjected to a 1-s air blast and cold water stimuli and their responses were recorded on a verbal rating scale. A total of 30 sites from 15 patients were divided into Group A-1.23% APF gel iontophoresis; and Group B-aqueous solution of hydroxyl-ethyl-methacrylate and glutaraldehyde. The teeth were evaluated immediately after the treatment and at the end of 2 weeks. In case of failure, the tooth was retreated with the same agent as before. Results: The results were statistically analyzed using Kruskal-Wallis analysis of variance and Mann-Whitney U tests. There was no statistically significant difference between the groups at the end of 2 weeks. However, Group A was more effective clinically, with fewer number of failures compared with Group B. Conclusion: Both the agents showed a statistically significant reduction in sensitivity compared with baseline; however, APF gel iontophoresis was more effective in reducing hypersensitivity over a longer time period.
